Uninsured or NON-SOS ADMITTED “FOREIGN” INSURANCE PLANS
$250 flat fee per visit
*Not all providers accept insurance. Ask prior to ordering. For insured patients a deductible may be required based on each patient individual plan. Patients will be responsible for the visit if their deductible has not yet been met. We also recommend that you check with your insurance carrier for the list of services that are covered under your plan.
Note:
The physician's fees include the initial visit and diagnosis. Any diagnostics or pharmaceutical services will be billed additionally.
Patients are provided with a comprehensive bill to submit to their health insurance carrier for reimbursement. Most major insurance will reimburse the visit up to 70%.
